Application for Membership
This is used for new officers to register their participation in the Skokie Police Pension Fund.

Application for Retirement
This is used to apply for your pension. Note that Disability Pension Applicants must also file the Application for Disability Pension Benefits).

Application for Disability Benefits
This is used for both duty and non-duty related disability applicants.

HIPAA Release for Physicians
This form may be needed by a physician to release disability information to the Pension Fund.

Physician's Certificate
This form is completed by physician's regarding an applicant's disability status.

Application for Withdrawal from Participation
This is used if you are leaving the Skokie Police Department and are not eligible or do not wish to apply for a pension benefit.

Request for Transfer of Creditable Service
Use this form to request transfer of service from another fund to the Skokie Police Pension Fund.
